Statutory Forms to be used in an application to the Tribunal

(N.B.) The T1A form is a 2 page form plus notes & requires a signature on the 2nd page before the Tribunal can process it.

T1-A Form

T1-A: This form is to be used for direct claims to the Tribunal under the following Acts:


T1-B Form

T1-B: This form is to be used for appeals of the Rights Commissioner’s Recommendation to the Tribunal under the following legislation:

T1-C Form

T1-C: Notice of Appeal to the Employment Appeals Tribunal under the Protection of Employees (Employers’ Insolvency) Acts, 1984 to 2004.


T1-D Form

T1-D: Notice for the Implementation of the Rights Commissioners Recommendation to Employment Appeals Tribunal under:


T1-E Form

T1-E: This form is to be used for appeals against a decision of the Minister for Enterprise, Trade and Employment or a Deciding Officer in a matter of redundancy under the Redundancy Payments Acts, 1967 to 2007.

T2: T2B: and T2D: These forms are to be used by a respondent against whom a claim has been lodged to enter a notice of appearance before the Tribunal. These forms are not available online, as each case is dealt with on an individual basis.
